1. Go to the applications link to the left, download the PDF application, fill out the necessary information and fax it to (614) 891-0535. You can click on the link in the PDF to go to the secure online payment form to set up your online profile and pay for the membership with a credit card to get access to BIA events and the members only section within 24 hours.
